About Us
Loretta Patrick founded SDCHL in June of 2006 .Her vision of providing free quality health care to the uninsured and low income became a reality as SDCHL opened it's doors for services in April 2007.
All that enter the doors of SDCHL will be forever grateful for Mrs.Patrick's vision and continued support .
MISSION
The Mission of The South DeKalb Center for Healthy Living is to provide quality primary and preventative healthcare services to DeKalb County Residents and surrounding areas; to address the health and social service needs of this population and to provide health education to the community at large.
VISION
The Vision of The South DeKalb Center for Healthy Living is to create a community culture where all strive for good health and well being; where illness and disease is prevented or controlled; and healthy living is the way of life.
GOALS
The Goals of The South DeKalb Center for Healthy Living are to:
-Prevent illness by performing health screenings, providing health education and encouraging life style modifications that promote wellness;
-Increase positive outcomes in patients with chronic diseases by providing regular screenings and aggressive case management;
-Provide High quality comprehensive primary care and preventive health services to those who could not otherwise afford it. Board of Directors
