We provide basic primary care,education & most labs at no cost
SDCHL is neither State or Federally funded and we have been blessed by many in order to continue to serve the community.
Due to tough economic times ….funding is becoming more difficult to secure.
In June 2010 we had to institute a Donation Request for each visit in order to keep the Clinic going. This donation is minimal as opposed to the actual cost of the services received. Services will not be refused for the inability to donate, however we need all of the community to Help us …Help Others
Starting June 2010 ..the Donation asked per visit will be
New & Return patients --$20.00 Donation per visit
(1st day thru 2nd year anniversary visit)
Return patients --$25.00 Donation per visit
(2nd year thru 4th year Anniversary visit)

Once a patient of the clinic and referred by Provider to the Specialty clinics above --- you WILL be called to schedule an appointment with the Hypertension Specialist or Gyn Specialist ***Appts for Chiropractic Services are made at Intake area ***
**GYN Services must be prescheduled and there is a small fee to cover costs of labs --$35.00 diagnostic Pap
